Three independent medical practices, three separate privacy notices.

Each doctor is a separate controller for their own patients’ data. The Gabinety Lekarskie Żółkowscy name only describes the shared location and offer; it does not identify a single controller.

Privacy notice

The applicable notice is the one for the doctor who treated you or will treat you. Please choose your doctor’s notice: Tomasz Żółkowski, Dorota Żółkowska, or Krzysztof Żółkowski.

All practices use the shared email address gabinetzielona@gmail.com. Please state which doctor your message is for. Do not send your full medical history or medical records in an unencrypted email.

Cookies and external services

This website stores your cookie choice in your browser. It also includes a Google map, the Medfile appointment calendar and links to external services. Please refer to each provider’s privacy information when using their services. Website statistics collection is currently inactive.

Terms of use

Terms and conditions for the provision of healthcare services. Effective from 1 January 2026.

§ 1 General provisions

1. These terms set out the rules for using healthcare services provided by Gabinety Lekarskie Żółkowscy with its registered office in Lubań at Zielona 7.
2. Use of the practice’s services constitutes acceptance of these terms.
3. Services are provided for a fee, in accordance with the price list available on the website and at the practice.

§ 2 Registration and appointment booking

1. Appointments can be booked:
   • By phone: +48 571 271 999 (aesthetic medicine, urology), +48 602 642 328 (surgery), +48 600 810 832 (ENT)
   • Via WhatsApp
   • In person at the practice
2. When registering, you must provide your first name, surname, PESEL number and contact telephone number.
3. The patient is obliged to arrive on time for the appointment.
4. A delay of more than 15 minutes may result in cancellation of the appointment.

§ 3 Cancellation of appointments

1. If you cannot attend an appointment, you must cancel it at least 24 hours in advance.
2. Appointments may be cancelled by phone or via WhatsApp.
3. Failure to cancel an appointment, or cancelling it late, may result in an administration fee at the next registration.

§ 4 Payments

1. The practice accepts the following payment methods:
   • Cash
   • Payment cards (Visa, Mastercard)
   • BLIK
2. The consultation is always included in the procedure price. Payment is made after the appointment.
3. Service prices are gross prices.
4. We issue invoices at the patient’s request. A request for an invoice must be made before payment is made..

§ 5 Scope of services

1. The practice provides services in the following areas:
   • General surgery – gastroscopy, rectoscopy, proctological procedures, removal of skin lesions
   • ENT – assessment and treatment of ear, nose and throat conditions
   • Aesthetic medicine – laser procedures, botulinum toxin, hyaluronic acid, platelet-rich plasma
   • Urology – assessment and treatment of urinary tract conditions

§ 6 Patients’ rights and obligations

Patients’ rights:

• The right to healthcare services meeting the requirements of current medical knowledge
• The right to information about one’s health condition
• The right to medical confidentiality
• The right to consent to or refuse a healthcare service
• The right to access medical records
• The right to submit comments and complaints

Patients’ obligations:

• Providing complete and truthful information about one’s health condition
• Following medical recommendations
• Arriving punctually for appointments
• Respecting the practice’s property and staff
• Paying amounts due on time

§ 7 Complaints

1. Complaints may be submitted:
   • In writing to: Zielona 7, 59-800 Lubań
   • By email: gabinetzielona@gmail.com
2. A complaint should include contact details, a description of the problem and the appointment date.
3. The complaint will be considered within 14 working days of receipt.
4. The response will be provided in the form in which the complaint was submitted.

§ 8 Contraindications and risks

1. Before the procedure, the patient will be informed about possible contraindications, risks and the course of the procedure.
2. The patient is obliged to inform the doctor about:
   • All medications being taken
   • Chronic conditions
   • Allergies
   • Pregnancy or suspected pregnancy
3. The procedure requires the patient’s written consent.

§ 9 Final provisions

1. In matters not covered by these terms, the following provisions apply:
   • of the Civil Code
   • of the Act on Patients’ Rights and the Patient Rights Ombudsman
   • of the Medical Activity Act
2. The controller reserves the right to amend these terms.
3. The current terms are always available on the website and at the practice.
